The two prevalent habits in our society: vaping and smoking have long-term ill-effect on the health of individuals and are pertinent on the eve of the World No Tobacco Day, May 31st, 2024. The habit of vaping in young generations has become a luxury among both men and women without understanding the consequences or long-term effects. People asking for a smoke deaddiction are seeing vaping as a better alternative rather than seeing a specialist. Both practices, vaping and smoking when scrutinized under scientific studies, have shed light on their detrimental health effects.

Vaping (e-Cigarettes): The Modern Menace

Proponents of vaping argue that it is a safer alternative to traditional flavourings and other chemicals. Studies have shown that vaping can cause inflammation and irritation in the lungs, organ damage and addiction. Long-term effects are not fully understood, but there is evidence of potential harm from substances that are used for vaping such as diacetyl, heavy metals and volatile organic compounds (VOCs) delivered to the lung. Today, many people who are resenting to vaping, are defending their choice as to be the safest choice of warding off nicotine without any scientific clues and spoiling their health slowly.

Smoking: The Burning issue

Smoking is a well-established health hazard. Tobacco smoke contains over 6000 chemicals, which are toxic and also carcinogenic. Smoking itself has been well aware as a potential risk factor that can lead to heart disease, stroke, chronic obstructive pulmonary disease (COPD) and various types of cancers. It affects almost all the organs in the body and is classified as the leading cause of preventable death.

Scientific Evidence:

A study published in “Respiratory Research” suggests that while e-cigarettes might be less harmful than conventional cigarettes, they still pose a great danger and their long-term effects are scarcely investigated. Another study funded by the National Institute of Health found that long-term use of e-cigarettes can significantly impair blood vessel function, increasing the risk of cardiovascular disease.

A Diabolic conclusion:

Both vaping and smoking carry long-term disadvantages that can impact the health. While vaping is often marketed as a safer alternative, the lack of long-term studies and the presence of harmful chemicals present significant risks. Smoking with its extensive research, continues to be a major health threat.

Hyderabad, 5th January 2021:

Aseptic Abscess syndrome is an autoinflammatory disease with fever, abdominal pain, and abscesses usually involving the spleen, liver; and associated with Inflammatory Bowel Disease in the majority of the cases. Cutaneous lesions are present only in 20% of the cases and are mainly cutaneous abscesses. It is often a relapsing condition and the CT scan may be an appropriate diagnostic tool in the setting of recurrent episodes of fever and pain in the abdomen. Lab tests like ESR, CRP are often elevated with elevated white blood count in basic blood pictures.

A 21-year-old woman Lakshmi (Name changed) from Khammam was suffering from a unique autoimmune disease, Aseptic abscess syndrome with pyoderma gangrenosum which has gone undiagnosed for a long time. She was given intensive antibiotic therapy but no response and advised removal of spleen in the hospitals where she visited earlier. She was then referred to Kamineni Hospitals, LB Nagar.

Doctors at Kamineni Hospitals had successfully treated this rare disease, Aseptic abscess syndrome with pyoderma gangrenosum. She was not given a solid diagnosis or treatment elsewhere for high fever, nodules and multiple ulcers over both hands and legs of sudden onset. On ultrasonography, multiple abscess in the spleen was noted.

“We had examined the patient’s condition with her complete medical history and did an ultrasound abdomen, skin biopsy, and basic blood tests. The patient had been diagnosed with a rare condition that affects the skin and internal organs severely called aseptic abscess syndrome with pyoderma gangrenosum. We have given her steroid injections under strict monitoring and liquidated her with immunomodulator tablets. Within 2 months, Abscesses in the spleen resolved but the cutaneous ulcers were resistant to tablets. Hence, we had started biological injections (newer therapies) sequentially under strict supervision with follow-ups, which helped with the overactive immune system that was acting against her and patient got back to her normal health status” said Dr. Ramadevi B, Consultant Dermatologist, Kamineni Hospitals, LB Nagar.

They were about to remove her spleen but we are extremely happy to salvage it, the puzzle as to why this young girl deteriorated all of a sudden was solved. We are beyond happy to develop an abundance of trust with this patient and her family” Dr Ramadevi B, Consultant Dermatologist, Kamineni Hospitals added further.

Rare Surgery at Kamineni Hospitals Gives New Lease of Life to 3-year-old Epileptic Child

Doctors at Kamineni Hospitals performed a rare surgery on 3-year-old Grace, who was taken to the hospital at LB Nagar by her parents with a developmental disorder, unusual episodes of laugh called Gelastic seizures. Gelastic seizures are usually present in childhood, even in the newborn period, and are manifested by frequent attacks of inappropriate laughter resulting from seizure activity. Studies show that Gelastic seizures are very rare and only one in every 200,000 children suffers from this abnormality.

Grace used to have bought of laughter i.e, suddenly, the child laughs for no obvious reason, situation or cause. The child was evaluated when she had abnormal laugh diagnosed to have gelastic seizure found to have sub centimetre lesion in the hypothalamus most probably hypothalamic hamartoma and started on anti-seizure medications. Parents went around the hospitals for treatment with a futile result.

6 months ago child used to get seizures once a month and it was lasting for 10 seconds, but now the frequency of seizures increased to 5- 6 times a day and increasing the duration of seizures to 1 minute. The child also developed a new-onset squint in the Left eye. In our hospital, she was evaluated with a team of doctors i.e, Neurosurgeon, Neuro physician, Paediatrics and Endocrinologists. She underwent imaging (MRI brain plain and contrast) in high end 3T MRI which showed a large lesion extending from hypothalamus (which is a part of the brain that houses a number of small nuclei that undertake a variety of functions) with compression on major vessels and nerves (especially to vision and extra ocular movements). Endocrinologists diagnosed the child to have reached puberty stage (corresponding to 8 years of age) because neurons in the tumour function as a heterotopic hormone (GnRH ) pulse generator.

“Child parents have been explained about the condition, rarity of disease requirement of surgery and risk involved. Parents were explained about other modalities of treatment. After thorough counselling, the child was taken for craniotomy and excision of the tumour using Navigation under general anaesthesia. Having Navigation in our armentarium helped us to do surgery in a minimally invasive manner without disturbing major structures especially nerves responsible for vision, extraocular movements and vessels. Post-surgery the frequency of seizures has significantly come down. ” said Dr. Ramesh Consultant Neurosurgeon, Minimal Access brain and spine surgeon, Kamineni Hospitals, Hyderabad.

“The most common areas of the brain which give rise to gelastic seizures are the hypothalamus that regulates the pituitary gland. A common cause of gelastic epilepsy is a small tumour in the hypothalamus. The majority of such tumours are benign. Children with hypothalamic hamartomas usually have gelastic seizures, cognitive malfunction, behavioural disorders. Early identification, proper diagnosis and management would help children to have better outcomes” Dr. Ramesh added.

The child’s parents had taken Grace to many hospitals but did not receive proper treatment. They are happy with the treatment given to Grace at Kamineni Hospitals.
